ZealCare aims to improve the health and well-being of people with complex chronic diseases. The company licenses Duke’s  Personalized Health Planning within Shared Medical Appointments (PHP-SMA)  program to care for Type II Diabetes and Type II Diabetes with Chronic Kidney Disease. Developed by the Duke Center for Personalized Health Care, the program is a proactive and participatory clinical approach to chronic care delivery which enables greater patient engagement with the added benefits of social support.
